Grant Request

Each year the United Way of Daviess County allocates money from their campaign to agency partners through a grant process.  You will find the forms and instructions needed to complete the grant request below.  The grant application is due on January 15, 2020.  If you are interested in applying, please click on each of the attachments below to fill out. You will need to submit 3 hard copies of all requested materials by Jan. 15th either to our location of 200 E. Van Trees, 2nd Floor, mail to our P.O. Box 224, or you can bring them to our next CROP meeting scheduled for Dec. 10th (if completed by then).

A couple small changes to note. We added "Impact Project Work" to one of the options to apply for, utilizing the same existing questions asked and we have moved the collaborative opportunity into its own grant request, also using the same questions from the original grant. This will be a $10,000 gift you can apply for and you can apply for both grants. The actual questions and grant material are almost identical as previous years.
